To register for our free prescription collection service you can either -

1) Print off and return to us a copy of our 'FREE PRESCRIPTION COLLECTION FORM' which can be downloaded by clicking here. This form can be viewed using Adobe Acrobat® Reader. If you haven't already got this then you can download it from the Adobe website by clicking here.

Request Prescriptions Online

We have an arrangement with the following surgeries to dispense repeat prescriptions to patients -

Dr Board & Partners (Court View Surgery)
Dr Ojagbemi & Partners (St Mary's Medical Centre)

This service is only available to patients of these surgeries who have a valid, up-to-date, repeat prescription.
